Why did I get a bill?

If you have received a bill, it is either because you didnt call the office 24 hours before your scheduled appointment to cancel or your co-insurance is the due for your previous visit.

 

What do I need to bring to my appointment.

Every patient needs to bring their medication bottles, copay / deducible amount, insurance card, and ID.

If you are a new patient you will also need to bring in a list of previous doctors with phone numbers and new patient pakcet.

 

How do I know how much I am suppose to pay for my visit?

Every patient will receive a call the day before letting them know the appointment time and amount due at time of service.

You may also check with your insurance company as well to verify this information.

 

Does our office do walk-ins?

No, everyone has to make a appointment ot be seen unless an emergency and approved by the doctor first.

 

How soon are we accepting new patients?

As soon as the next working day.
